In Palm Beach County, your child will not be able to start school without the proper vaccinations and documentation. Waiting until the last minute could mean long lines and wait times. Avoid the end-of-summer rush and get your child vaccinated now!

In fall 2019, my child is going into...

Before entering or attending school in Florida (kindergarten through twelfth grade), each child must provide a Florida Certification of Immunization (DH 680 form), documenting the following vaccinations:

  • Four or five doses of DTaP
  • Four or five doses of IPV
  • Two doses of MMR
  • Three doses of Hep B
  • Two doses of Varicella. Varicella vaccine is not required if there is a history of varicella disease documented by the health care provider.

FOR INCOMING KINDERGARTNERS: documentation of a health examination (physical) is also required.

  • Four or five doses of DTaP
  • Four or five doses of IPV
  • Two doses of MMR
  • Three doses of Hep B
  • Two doses of Varicella. Varicella vaccine is not required if there is a history of varicella disease documented by the health care provider.

If your child is new to Palm Beach County, documentation of a health examination (physical) is also required.

Before entering or attending school in Florida (kindergarten through twelfth grade), each child must provide a Florida Certification of Immunization (DH 680 form), documenting the following vaccinations:

  • Four or five doses of IPV
  • Two doses of MMR
  • Three doses of Hep B
  • Two doses of Varicella (kindergarten effective with 2008–2009 school year, then an additional grade is added each year thereafter). Varicella vaccine is not required if there is a history of varicella disease documented by the health care provider.
  • One dose of tetanus-diphtheria-pertussis (Tdap) vaccine in grades seven.
  • An updated DH 680 form to include Tdap, must be obtained for submission.

FOR INCOMING 7th GRADERS: documentation of a health examination (physical) is also required.

Before entering or attending school in Florida (kindergarten through twelfth grade), each child must provide a Florida Certification of Immunization (DH 680 form), documenting the following vaccinations:

  • Four or five doses of DTaP
  • Four or five doses of IPV
  • Two doses of MMR
  • Three doses of Hep B
  • One Tetanus-diphtheria-acellular pertussis (Tdap)
  • Two doses of Varicella. Varicella vaccine is not required if there is a history of varicella disease documented by the health care provider.

If your child is new to Palm Beach County, documentation of a health examination (physical) is also required.

NOTE: The fifth dose of DTaP vaccine is not necessary if the fourth dose was administered at age 4 years or older. If four or more doses are administered before age 4 years, an additional dose should be administered at age 4 through 6 years and at least six months after the previous dose. A fourth dose is not necessary if the third dose was administered at age 4 years or older and at least six months after the previous dose. Varicella vaccine is not required if varicella disease is documented by the health care provider.
